Vitale Education: Graduated from St. Joseph's Hospital
School of Nursing, 1994 Experience:
My experience is primarily in Medical/Surgical
nursing and Geriatrics. I have been involved in
every facet of home health care from admission to
discharge and beyond. I believe it is important to
continue contact with patients that may need
additional assistance after
discharge. Bio:After spending 15 years as a
nurse and working closely with patients and their
families, I noticed that there was a gap in the
circle of their health care. It seemed once the
patient was discharged from the hospital or left
the office, they were no longer being cared for. I
believe this is where the break down begins and
the gap forms between a patient and their health
care providers. This often results in unanswered
questions, re-scheduled appointments, and in some
cases re-admissions into the hospital because they
did not understand the discharge orders, or were
unable to go to follow-up appointments. In other
cases, patients simply had difficulty
understanding diagnoses, clinical terminology, and
what was involved in their plan of care. I am
committed to bridging that gap in order to improve
patients’ understanding of and involvement in
their care. I believe this will improve patients’
overall health and help preserve their
independence and dignity.
Phyllis Chiodo, RN
(Retired) Education: Graduated from Church Home & Hospital,
Baltimore, MD in 1960, 40 years OB-GYN experience,
with experience in day surgery. Experience:
I have over 40 years OB/GYN experience as well as
experience with Same-Day Surgery. Bio: For 40
years I participated in one of life's greatest
experiences, the birth of a child. I was able to
share with my patients, one of their happiest and
most memorable experiences. I retired very
satisfied and enjoyed being a part of such a
blessing. I also consider my experience as a
mother, grandmother and patient to be very
valuable in helping others. I have found that
drawing from my professional and personal
experiences together, I am able to provide a
better level of care and
understanding.
Jennifer Vitale, RN,
BSN Education: Graduated from FSU in 1992 with my BSN and
continued on to receive my RN at IRCC in Fort
Pierce, FL in 1999. Experience: I have 7
years experience in Emergency Medicine. Bio: I work
primarily in the Emergency Room at Martin Memorial
Hospital where every day is a challenge and every
case is unique. My satisfaction comes from knowing
I am contributing to improve a patient’s health
and quality of life.
